Below is a fillable medical form which can be used to assist responding EMS units in the event of a medical emergency. This data is NOT stored with the Garden City Park Fire Department and is designed to be printed by you. Keep this form in a quickly accessible place (We suggest putting them in a plastic sleeve and attaching to the refrigerator with a magnet).
Please complete one form for each member of your family. Keep the form(s) in easily accessible places!
We suggest placing in a plastic sleeve and attach it to the fridge with a magnet for Police and or Fire Department /
EMS personnel.
If DNR or MOLST form is active, the original paperwork must be readily available for EMS personnel otherwise, EMS personnel will follow normal protocol.
